Tomorrow is the big day. I am a bit nervous but today showed that we are in form. We cycled 65k and did 3 small mountains. Suzanne joined us on a bike, she really has the ability to become a great rider.
The group is great, we have despite my nervousness a lot of laughs. I'll try to post something have way.

I am ready!
Dinsdag is het zover. Ik ben er klaar voor. Ik heb er genoeg voor getraind. Nu maar hopen dat ik goeie benen heb.
Afgelopen week nog een gastroscopie gehad. Ze kijken dan met een in je slokdarm en maag. Ze hebben ook weer weefsel stukjes genomen. De plek waar mijn oorspronkelijke tumor zat was nog te zien, maar was nog kleiner geworden. Je zou er overheen kunnen kijken als je niet zou weten waar op moet letten. Het was volgens de dokter uniek dat de plek nog meer geslonken was sinds November. Het geeft de burger nog meer goeie moed. Ik zit op de goeie weg. Wie had dat een jaar geleden durven dromen! Mijn arts heeft me nog wel gewaarschuwd dat ik niet helemaal tot het gaatje moet gaan op de Mont Ventoux. Ik ga mijn strategie aanpassen en niet meer voor een tijd binnen de 2 uur maar voor een geslaagde poging om hem 2 keer te doen, terwijl ik daarna nog lachend op de foto kan!
Na een jaar begint mijn arts mij goed te kennen. Zo goed dat de zusters zich afvroegen of we elkaar ergens van kende. Fijn is dat. Ergens na de zomer zal ze een praatje houden over onderzoek naar slokdarmkanker. Iedereen die mij gesteund heeft is uitgenodigd. Ik zal haar die middag een cheque overhandigen...hoe groot die cheque zal zijn hangt van jullie en mij af...jullie, voor diegene die nog niet gestort hebben, mij, of ik het twee keer ga halen en dus de verdubbelaar verdien. De cheque zou zomaar meer dan Euro 50,000 groot kunnen zijn, dat hangt van JULLIE af. Ik ben jullie hiervoor ongelofelijk dankbaar.
Mijn kanker jubileum hebben we gevierd. Mijn zusje en paar goeie vrienden hadden een verrassingsdinnertje geregeld. We wisten van niets, ik zou naar Maastricht gaan om te trainen maar werd opeens ontvoerd samen met Suus. Na een half uur met blinddoek door het Gooi rijden bleken we we weer thuis te zijn. Een ontvangst alsof we een Olympisch stadion ingingen. Energy! Het was een fantastische avond; iedereen had een filmpje gemaakt, dat door een professionele vriend in een mooi stuk was geëdit. Het was gezellig maar ook heftig. Het blijft raar om je kankerjubileum te vieren. Maar het voelde goed. Suzanne en ik waren beiden er door overdonderd en geroerd. Ik heb nog als een echte schrijver mijn laatste blog voorgelezen. Deze sloot naadloos aan bij het gevoel dat we die avond hadden. Mijn vrienden uit Singapore hadden op eigen initiatief ook een filmpje gemaakt. Hoe toevallig, maar oh zo leuk.
We gaan proberen jullie dinsdag op de hoogte te houden van mijn vorderingen......Ik heb er in iedere geval zin in. Ben een beetje gespannen maar dat zal zich wel omzetten in fietsenergie. De beuk erin.

Tuesday is the day. I'm ready. I've training enough for it. Let's hope my legs feel good.
I had a gastroscopy last week. They go with a camera to look at your oesophagus and stomach. They also took some tissue samples. The place where my original tumor was was still visible, but had become even smaller. According to the doctor you could look over it if you would not know where to look for. It was according to the doctor unique that it had shrunk even more since November. It gives me good courage. I'm on the right track. Who would have dreamed of that a year ago! My doctor has warned me that I should over do it on the Mont Ventoux. I'm going to change my strategy and will not do my best for a time within 2 hours but will focus on completing it twice while I still can smile on the picture!
After a year my doctor begins to know me well. So good that the sisters wondered if we knew each from somewhere else. That is great. Sometime after the summer my doctor will hold a talk about research on esophageal cancer. Anyone who supported me is invited. I will hand her over a cheque that afternoon ... how big that cheque will be depends on you and me ... you, for those who have not yet supported; me, whether I will be able to complete it twice and get the doubler . The amount currently is just over 50,000 Euro; it might even be higher depending on YOU. I am incredibly grateful to you for the support.
We celebrated my cancer anniversary. My sister and some good friends had a arranged a surprise dinner for us. We were aware of nothing, I would go to Maastricht to train but was suddenly kidnapped along with Suus. After half an hour with a blindfold on driving through our region we were back at home; where we received a standing ovation as entering an Olympic stadium. Energy! It was a fantastic evening, everyone had made a short movie, which was edited in one cool format, it was cozy but also very intense. It's still strange to celebrate your cancer anniversary but it felt good. Suzanne and I were both overwhelmed by it and stirred. As a real writer I read my last blog out loud. It fitted very much seamlessly into the feeling that we had that night. My friends from Singapore had also made a film on their own initiative. How coincidental, but oh so fun.
We will try to update you on my progress on Tuesday...... in any event I am ready. I 'm a little tense but I will convert into energy on my cycle. Bring it on.

Dankbaarheid en hoop - Gratitude and hope
Zaterdag is het 1 jaar geleden. 1 jaar geleden dat mijn/ons leven onderste boven werd gekeerd; in volle vaart een plotselinge noodrem. Een haarspeldbocht in ons leven die wij niet voorzien hadden met een onmiddellijke U-turn terug naar Nederland. Daar keerden we terug naar de basis en kregen we voorzichtig hoop dat ik het beter dan de statistieken zou gaan doen. Het betekende het einde van ons ongecompliceerde leven.
Dit is een speciale blog. Een blog waarvan ik 1 jaar geleden niet zeker was deze te kunnen schrijven, maar waarvan ik wel overtuigd was dat ik er alles aan ging doen om het te halen. Ik heb het gehaald, en hoe. De eerste 'anniversary' van mijn kanker. Dat er nog velen mogen volgen. Ik besef nu pas dat als mensen mij vragen hoe oud ik ben, ik altijd 38 zeg, terwijl ik in februari 39 geworden ben. Mijn normale leven is stil komen te staan op mijn 38ste; om precies te zijn na 38 jaar en 128 dagen. Mijn kankerleven begon daarna, op 15 juni 2012. Sindsdien staat mijn echte leven stil en ben ik 38 en een kankerpatiënt voor altijd.
Een jaar geleden zat ik op een boot met 10 gasten in de golf van Phuket. Er is geen schriller contrast dan het moment dat ik nog van niets wist en het moment na mijn gesprek met de doctor. "Yes, you have to worry"; zei de dokter.
Ik heb afgelopen week alle gesprekken met artsen die we in de beginfase gesproken hebben opnieuw afgeluisterd (We namen met onze I-phone alle gesprekken op, met het idee om deze daarna rustig nog een keer af te kunnen luisteren. Dat hebben we natuurlijk nooit gedaan; wie wil er nou 2 keer horen dat je zeer waarschijnlijk binnenkort doodgaat?). Ik wilde eigenlijk jullie laten horen hoe het is om het bericht te krijgen dat je nog kort te leven hebt; helaas is de kwaliteit niet goed genoeg en moet je erbij zijn geweest (beeld hebben) om het gevoel goed over te brengen. Volgende keer een Go-Pro op mijn voorhoofd binden ;-). Het was confronterend om de gesprekken opnieuw af te luisteren maar ergens ook bevrijdend. Ik heb de eerste mijlpaal gehaald. Het slikken van de chirurg tijdens het eerste gesprek, als ik hem vertel over de knobbel in mijn nek, is erg heftig. Je merkt aan hem dat hij tot dat moment nog dacht dat de tumor plaatselijk was…helaas. Met spoed werd ik doorverwezen naar de oncologie-afdeling. En vervolgens het gesprek met de uitslag van de CT scan is het meest confronterend, omdat dan echt duidelijk wordt hoe erg de situatie is; nl. fase 4 waarbij operaties en bestraling geen zin meer hebben.
Een paar dingen vielen me mij bij het beluisteren van de gesprekken:
1. Ondanks het slechte nieuws bleven we relatief rustig
2. Vanaf het begin proberen we positief te zijn en geloven we niet in statistieken
3. Hoe efficiënt is Singapore; dat je op dag 1 alles weet en op dag 2 al met chemo kan beginnen. Het had weinig gescheeld of ik was inderdaad op dag 2 begonnen met chemo.
4. Wat waren we blij dat ondanks de extensie van mijn uitzaaiingen mijn organen schoon waren.
5. Wat is Singlish toch een rare grappige taal!
We zijn nu een jaar later. Wat voor gevoel geeft me dat? Dankbaarheid en hoop. Dankbaar dat ik het tot hier heb gehaald. Dankbaar dat ik Suus naast me heb. Dankbaar voor haar steun en die van de kinderen. Dankbaar dat ik de verjaardagen en partijtjes van Oene zijn 5de, Eline haar 6de en Fleur haar 8ste en Suus haar 40ste heb mogen meemaken. Dankbaar dat de kinderen goed geland zijn. Dankbaar dat we ons thuis in Bussum voelen en voorzichtig vrienden aan het maken zijn. Dankbaar dat er een plek op school en andere faciliteiten voor hen is gecreëerd. Dankbaar voor de hulp en band van/met mijn ouders en schoonouders, onze broers en zussen, onze 'close friends', en onze oude, nieuwe en overzeese vrienden. Dankbaar voor de steun van vrienden, familie en onbekenden. Dankbaar voor de aandacht van iedereen. Dankbaar voor de steun voor mijn Stay Strong actie (voor diegene nog mee moeten doen - graag, doen, alle beetjes helpen - nog even een we gaan door de Euro 40,000 limiet heen!!!!!). Dankbaar voor de baan die mijn werkgever gecreëerd heeft. Dankbaar voor de tijd en aandacht van secretaressen, bazen en andere collega's. Dankbaar voor de juiste, persoonlijke en fijne zorg van mijn dokter, de nurse practitioner en andere medewerkers van het AVL. Dankbaar voor de fijne sfeer die in het AVL heerst. Dankbaar dat ik niet ziek ben geweest van de chemo. Dankbaar dat de bijverschijnselen mee vielen/vallen. Dankbaar dat ik weer haar heb. Dankbaar dat ik zo veel heb kunnen en kan sporten. Dankbaar voor de 27 dagen skiën. Dankbaar voor de mooie reis naar Rome. Dankbaar voor de top vakantie in de US. Dankbaar voor het geluk dat de chemo bij mij goed lijkt te werken. Dankbaar voor het mooie jaar. Dankbaar dat ik hier nog ben. Dankbaar voor wat komen zal en hoop dat ik nog heel lang mee ga. Op naar mijn tweede kanker 'anniversary'. Dat er nog velen mogen volgen.
Voor diegene die het nog niet begrepen hebben mijn laatste CT scan en bloedwaardes waren weer onveranderd goed; op naar de volgende scan over 9 weken. Nu genieten van de zomer met zo nu en dan een ijkpunt (bloedonderzoek 2x en gastroscopie). Ik heb er vertrouwen in maar blijf vigilant.
Nu hard trainen voor de Mont Ventoux. Ik hoop dat het weer meezit en het me lukt om het 2 keer te doen op de 2de juli. Ik heb er zin in. De beuk erin.

Saturday it will be one year ago. 1 year ago that my / our life dramatically changed, at full speed, a sudden emergency brake. A hairpin in our lives that we had not foreseen, with an immediate U-turn back to the Netherlands. There we went back the basics and we cautiously got hope that I would do it better than the statistics It marked the end of our uncomplicated live.
This is a special blog. A blog which I was not sure I would be able to write a year ago, but which I was convinced that I was going to do everything in my power to do. I made it, and how. The first 'anniversary' of my cancer. That many more may follow. I now realize that when people ask me how old I am, I always say 38, while I became 39 in February. My normal life has come to a stop when I was 38, to be precise after 38 years and 128 days exactly. My life with cancer began on June 15, 2012. Since then my real life was over and I'm 38 and have cancer forever.
A year ago I was on a boat with 10 guys in the gulf of Phuket. There is no starker contrast than the time when I knew nothing about it and the time after the doctor's call. "Yes, you have to worry", he said doctor.
Last week I listen again to all conversations with doctors we spoke with in the beginning (We recorded all our phone calls with an I-phone; the idea was to be able to listen back to all conversations afterwards. I never did so, who wants to hear for a second time that you will probably die soon?). I actually wanted to let you hear what it's like to get the news that you have only a short time to live; unfortunately the quality is not good enough and you must see it also to have the correct picture. Next time I need to fix a Go-Pro on my forehead ;-). It was confronting to listen to the conversations again but somehow it was liberating. I have reached the first milestone. The swallowing of the surgeon during the first interview, after I told him about the lump in my neck is very painful to hear. Until that time it is clear that he thought that the tumor was only locally ... unfortunately. Urgently I was referred to the oncology department. The conversation with the results of the CT scan is the most confrontational, because it became clear how bad the situation really, ie phase 4; as a result of which surgical operation and radiation made no sense.
A few things caught my attention when listening to the calls:
1. Despite the bad news we were relatively balanced
2. From the beginning we tried to be positive and we did not believe in statistics
3. Singapore is so efficient! You know everything on day 1 and on day 2 you can already start with chemo. I had almost indeed started chemo on the 16th of June.
4. We were so happy that despite the extension of my metastases my organs were clean.
5. Singlish is a weird and funny language!
We are now a year later. What kind of feeling does that give me? Gratitude and hope. Grateful that I have reached this time. Grateful that I have Suus beside me. Grateful for her support and that of the children. Grateful that I was able to celebrate the birthdays of Oene, his 5th, Eline, her 6th, Fleur he 8th and Suus, her 40th. Grateful that the children landed well. Thankful that we feel at home in Bussum and carefully are making new friends. Grateful that the children were accepted in school and other facilities. Grateful for the help and band with our parents, our siblings, our 'close friends', and our old, new and overseas friends. Grateful for the support of friends, family and strangers. Grateful for the attention of everyone. Grateful for the support for my Stay Strong action (for those have who not yet joined - do it, every little bit helps - with a bit more support we will reach the Euro 40,000 limit!!). Grateful for the job that my employer has created. Grateful for the time and attention of secretaries, bosses and other colleagues. Grateful for the personal and delicate care of my doctor, nurse practitioner and other staff of the AVL. Grateful for the friendly atmosphere that prevails in the AVL. Grateful that I was not sick from the chemo. Grateful that the side effects were/are not to painful. Grateful that I have my hair back. Grateful that I have been able and am to exercise. Grateful for my 27 days of skiing . Grateful for the beautiful trip to Rome. Grateful for the big holiday to the U.S.. Grateful that the chemo seems to work well for me. Grateful for the great year. Grateful that I'm still here. Grateful for what is to come and hope that I will last for long. Looking forward to my second cancer 'anniversary'. That many more may follow.
For those who have not understood it yet my last CT scan and blood levels were again unchanged and thus positive, on to the next scan over 9 weeks. Now ready to enjoy the summer with occasionally a benchmark (2x blood test and endoscopy). I am confident but stay vigilant.
Now training hard for the Mont Ventoux. I hope the weather is good and that I will manage to do it twice on July 2nd. I am ready. Bring it on.

Grateful that I'm still here. Grateful for what is to come and hope that I will last for long. Looking forward to my second cancer 'anniversary'. That many more may follow.
For those who have not understood it yet my last CT scan and blood levels were again unchanged and thus positive, on to the next scan over 9 weeks. Now ready to enjoy the summer with occasionally a benchmark (2x blood test and endoscopy). I am confident but stay vigilant.
Now training hard for the Mont Ventoux. I hope the weather is good and that I will manage to do it twice on July 2nd. I am ready. Bring it on.

Spannend en ontspanned - Tension and relaxation
Vandaag CT-scan gehad, maandag uitslag, fingers crossed dat het weer een goeie uitslag is! Wat de uitslag ook is ik ga fietsen, ik moet wel met zoveel steun van mensen voor mijn actie. Ik ben stil van het succes van de actie, vooral omdat er zo veel mensen zijn die me steunen. Het is heel speciaal om steun te krijgen van mensen die ik niet ken. Het is alsof mijn verhaal er toe doet en mensen raakt. Daarnaast voel ik me weer eens nuttig. Ik doe iets terug. Denk erom, alle beetjes helpen en geven me nog meer stimulans om omhoog te fietsen!
De tijd na de scan voelt angstig. De foto's zijn gemaakt, er is niets meer aan te doen. Dankzij Suzanne heb ik geleerd om te genieten tot de uitslag, in plaats van de hele tijd te stressen voor de uitslag....dat lukt natuurlijk niet altijd, ongetwijfeld dat ik een aantal slaappillen zal moeten nemen. De komende dagen zijn vol gepland met leuke activiteiten. Een aantal ex-Singaporesen komen de opening van de Cold Storage en Swiss Butcher in Bussum vieren zaterdagavond! Morgenochtend golven en komende dagen nog ff 80km fietsen. Mocht het nog niet duidelijk zijn de beuk gaat erin tot maandagochtend, maar ook zeker erna. Het moet!

I'm touched by the success of the charity ride, especially since there are so many people who support me. It is very special to get a donation from people I do not know. It's like my story is relevant and touches people. In addition, I feel useful again. I am doing something back. Remember, hether small or big, all donations help and give me even more incentive to conquer the Mont Ventoux!
The time after the scan feel anxious. The photos are taken, there is nothing more to do. Thanks to Suzanne, I have learned to enjoy the time before the result, instead of stressing all the time until the results .... that does not always work, the chances are that I will have to take some sleeping pills. The next few days are filled with fun activities. Some ex-Singaporean are coming over to celebrate the grand opening of the Cold Storage and Swiss Butcher in Bussum Saturday night! Tomorrow a round of gold and during the next few days 80km of cycling. If it is not yet clear I am ready for monday and will do every I can until and after that day!

Stay Strong - Sponsoractie Slokdarmkanker / Charity for Oesophageal Cancer
Begin juli
is het iets meer dan 1 jaar geleden dat bij mij slokdarmkanker met uitzaaiingen
naar hals en buik (stadium IV) werd geconstateerd. Na een turbulent jaar,
verhuizing naar Nederland, zware chemo, onderhouds-chemo, ben ik fit genoeg om
op 2 juli de Mont Ventoux op te fietsen.
Omdat
jullie mij op mijn blog volgen nodig ik jullie uit om mij naar boven te
sponsoren. Hoe meer geld ik ophaal hoe harder ik zal fietsen……beloofd! Ik heb
eindeloos gedacht over allemaal formules om het aantrekkelijker en spannender
te maken, en ook natuurlijk om meer geld in te zamelen, maar ik denk dat het
toch te ingewikkeld wordt en dat het moeilijk zal zijn het sponsorgeld
inderdaad te ontvangen; vandaar een simpele approach. Ik vraag je mij te
sponsoren voor een nominaal bedrag. Dit bedrag mag zo groot zijn als jezelf
wil. Daarnaast is er de optionele verdubbelaar: als het mij lukt de Mont
Ventoux twee keer op te fietsen in 1 dag verdubbel je het nominale bedrag.
Hoewel ik begrijp dat het volgens de pers en het CPB crisis is in Europa vraag
ik jullie om toch je portemonnaie te openen en deze week misschien geen
biefstuk te eten maar een goedkope vegetarische maaltijd. Voor de Singaporesen
en andere Aziaten, daar waar het geen crisis is, misschien een weekend geen
enkele keer uit eten maar gewoon thuis blijven het hele weekend (ahhh)....Het
maakt niet uit hoe groot de gift is maar ik hoop wel ten minste EUR 35,000 bij
elkaar te leuren (doel altijd bijstellen als nodig of uitdagend te houden). Daarnaast moet je je ook realiseren dat dit ook jou, je
man/vrouw of kind had kunnen of nog steeds kan overkomen. Geld voor onderzoek
is belangrijk en kan en zal levens redden. Denk daar maar over na als je
jouw sponsorbedrag bepaalt.
Slokdarmkanker
Bij
slokdarmkanker is er sprake van een kwaadaardige tumor in de slokdarm. In de
medische wereld worden vaak de namen slokdarmcarcinoom en oesofaguscarcinoom
gebruikt. De medische naam voor slokdarm is ‘oesofagus’. En ‘carcinoom’ is een
ander woord voor een kwaadaardige tumor of kanker. Slokdarmkanker
komt in Nederland bij 5,8 mensen op de 100.000 voor. In Zuid-oost Azie is dat
iets lager 3,6 mensen per 100.000. Slokdarmkanker komt steeds vaker voor bij
jongeren (~40 jaar) mensen. Helaas wordt de ziekte bij ongeveer
de helft van de patiënten, zoals bij mijzelf, pas in een vergevorderd stadium ontdekt.
De behandelingsmogelijkheden zijn in dat geval beperkt. In een gevorderd
stadium is meestal alleen een palliatieve behandeling mogelijk. Dat betekent
dat de behandeling niet meer gericht is op genezing. Een palliatieve
behandeling is bedoeld om de ziekte zoveel mogelijk af te remmen, en de
klachten te verminderen. Als slokdarmkanker in een vroeg stadium wordt
vastgesteld is een curatieve behandeling vaak nog mogelijk. Een curatieve
behandeling is een behandeling die gericht is op genezing en bestaat meestal
uit een operatie, soms in combinatie met bestraling of chemotherapie. De mate
van metastase (uitzaaiingen) wordt aangeduid in stadia; als de tumor
beperkt is tot de slokdarm is dat stadium I en II; bij lokale uitzaaiingen is
het stadium III en bij meer gevorderde uitzaaiingen, zoals bij mij, stadium IV. Gemiddeld
gezien is de overlevingskans van patiënten dat een curatieve behandeling
ondergaat na vijf jaar ongeveer 25 tot 30 %. De vijfjaarsoverleving van alle
slokdarmkankerpatiënten samen is ongeveer 10%. Dit betekent dat de
vijfjaarsoverlevingskans van patiënten met een palliatieve
behandeling heel erg laag is (<2%). Om mij als voorbeeld te gebruiken: Ik
had volgens de statistiek, gezien de metastase van mijn tumor naar buik en
hals, nog 3 tot 6 maanden te leven zonder chemo en 8 tot 11 maanden met chemo.
De precieze
oorzaak van slokdarmkanker is onduidelijk. Een van de doktoren die ik
sprak noemde het een welvaartsziekte, veroorzaakt door de manier waarop we
allemaal leven. Waarschijnlijk spelen verschillende factoren gecombineerd een
rol bij het ontstaan van deze vorm van kanker. Van een aantal factoren is
bekend dat zij de kans op slokdarmkanker vergroten. Deze zijn onder andere:
Roken en
overmatig alcoholgebruik;
Vooral de combinatie van deze twee factoren verhoogt
de kans om slokdarmkanker te krijgen.
Ongezonde
en eenzijdige voeding;
Het eten van weinig groenten en fruit lijkt de kans op
slokdarmkanker te vergoten.
Overgewicht;
Regelmatig
en langdurig terugstromen van maagzuur in de slokdarm;
Dit wordt ook wel
brandend maagzuur of reflux genoemd.
Zoals ik
weleens eerder heb geschreven is de grootste factor gewoon domweg pech.
Het geld
dat ik inzamel zal 100% gebruikt worden voor onderzoek naar slokdarmkanker in
het AVL (Anthony van Leeuwenhoek ziekenhuis), het ziekenhuis waar ik onder
behandeling ben, waar ik gezamenlijk met de doktoren bezig ben een nieuwe Gauss
kromme neer te zetten, waar ik de statistieken niet alleen ga verslaan maar ga verbreken.

De Mont
Ventoux is een van de bekendste bergen in wielerland. Er zijn 3 manieren om de
Mont Ventoux te beklimmen. De bekendste klim start uit Bédoin en is
21.8km lang. Je moet 1670m stijgen tot de top op 1912m hoogte. Het
gemiddelde stijgingspercentage is 7.4% met stukken van 11% er tussen. De
snelste tijd is door Iban Mayo gereden (55.51 minuten). De gedrogeerde Lance
Armstrong reed er in zijn EPO hoogtijdagen 58m over. Ik wil hem binnen 2 uur
fietsen en ik wil hem tenminste 2 keer doen op 1 dag, 1 keer vanuit Bédoin en 1
keer 1 van de andere 2 routes. Gelukkig ga ik dit avontuur niet alleen aan maar
mijn kleine zusje en oudere broer, de man "uit Singapore die naar NY
gaat", de "Oostenrijk specialist" en wellicht ook een van
mijn zwagers (als hij het aandurft) gaan mee.

Early July
it will be just over one year ago that I was diagnosed with esophageal cancer
with metastases to neck and abdomen (stage IV). After a turbulent year, moving
to the Netherlands, heavy chemo, maintenance chemo, I am now fit enough to
cycle up the Mont Ventoux on July 2nd.
Because you
follow me on my blog I invite you to sponsor me. The more money is donated the
quicker I will cycle ...... promised! I have endlessly thought about all kind
of formulas to make it more attractive and exciting and of course to raise more
money, but I think it’s easy to get too complicated. Therefor I have kept it
simple; I ask you to sponsor me for a nominal amount. This amount may be as
large as you want. There is also the optional multiplyer: if I manage to cycle
the Mont Ventoux twice in one day you double your donation amount. While I understand
that according to the press and the statistics bureau it is crisis in Europe, I

steak this week. For the Singaporeans and other Asians, where it is not a
crisis nut sky is still the limit, maybe a weekend with no eating out in fancy

Oesophageal
cancer
I you have
oesophageal cancer there is a malignant tumor in your oesophagus. In the
medical world this is called an oesophageal carcinoma. On average 5.8 people
per 100,000 get Esophageal cancer in the Netherlands. In South-East Asia the
average is slightly lower 3.6 per 100,000 people. Esophageal cancer is
increasingly common among young people (~ 40 years) people. Unfortunately, for
about half the patients the disease is only diagnosed in an advanced stage
(like in my case). The treatment options are limited in that case. In an
advanced stage only palliative treatment is possible. This means that the
treatment is no longer focused on healing. A palliative treatment is intended
to slow the disease as much as possible and reduce the symptoms. If esophageal
cancer is diagnosed at an early stage, a curative treatment is often still
possible. A curative treatment is a treatment aimed at cure and usually
consists of surgery, sometimes in combination with radiation or chemotherapy.
The degree of metastasis (spread) is referred to in stages, when the tumor is
limited to the esophagus it is called stage I or II, in case of local
metastases stage III and in case of advanced metastases, such as in my case,
stage IV. On average, the survival rate after five years of patients that undergo
a curative treatment is about 25 to 30%. The five-year survival of esophageal
cancer for all patients together is about 10%. This means that the five-year
survival rate of patients with palliative treatment is very low (<2%). To
use me as an example: according to the statistics, given the metastasis of
tumor to my stomach and neck, I was given only 3 to 6 months to live without
chemo and 8 to 11 months with chemotherapy.
The exact
cause of esophageal cancer is unclear. One of the doctors I spoke to called it
a lifestyle disease, caused by the way we all live. Probably several factors
play a combined role in the development of this cancer. A number of factors are
known to increase the risk of esophageal cancer. These include:
• Smoking
and excessive alcohol consumption;
The combination of these two factors
increases the risk of developing esophageal cancer.
• Unhealthy
and unbalanced diet;
eating enough fruits and vegetables
seems to shed the risk of oesophageal cancer.
•
Overweight;
• Regular
and prolonged reflux of stomach acid into the oesophagus;
This is also called heartburn or reflux.
• As I have
written before, often the biggest factor is just bad luck.
The money I
will collect will be used for a 100% for research on oesophageal cancer in the
AVL (Anthony van Leeuwenhoek hospital), the hospital where I was treated, where
together with the doctors I am not beating the statistics and developing a new
Gaus curve but where I am breaking the statistics. Stay strong, I will beat the

The Mont
Ventoux is one of the most famous mountains in the road cycling arena. There
are three ways to ascend it. The most famous climb starts from Bedoin and is
21.8km long. You must 1670m rise to the top at 1912m altitude. The average climbing
percentage is 7.4% with stretches of 11%. The fastest time is was cycled by
Iban Mayo (55.51 minutes). The drugged Lance Armstrong rode it in his EPO
heyday in about 58m. I want to cycle it within two hours and I want to do it at
least two times in one day, one time starting from Bédoin and one time starting
from one of the other starting points. I'm happy not to do this adventure by
myself but being accompanied by my little sister and older brother, the man

Na zonneschijn toch regen in mijn hoofd - After sunshine rain in my head
Ik had afgelopen dinsdag weer goede bloedwaardes. Dat voelde goed. Ja, je leest het correct - verleden tijd.....sinds daarna voel ik me anders. Dinsdagnacht heb ik vreselijk gezweet. Op woensdag las ik toevallig dat nachtzweet een kenmerk is van lymfenactiviteit.
I had good blood levels again last Tuesday. That felt good. Yes, you read that correctly - past tense..... since then I feel different. Tuesday night I sweat terribly. On Wednesday I read that night sweats is a characteristic of lymphatic activity.
In addition, June 15 is getting awfully close. 1 year ago. That is not easy. I'm already thinking about doing a blog for around that time. A killer blog. June 6th, I will have a new CT scan. June 10th I will receive the results. I feel the tension now already; far too early. I long for the feeling I had in America. No stress, no fear.
Another factor that gives me stress is that for the first time since getting ill I am surfing the internet for information on esophageal cancer. Why? Because you will be sponsoring me up the Mont Ventoux. Proceeds will go to esophageal cancer research. But if you sponsor me, I need to be able to tell you what esophageal cancer is, who gets, what are the statistics. The figures give me a really depressed feeling, they are so negative.
I just also experienced what it is like to be on the other side of a disaster. Someone in my vicinity was possibly diagnosed with cancer. The person had have a CT scan done as the referral mentioned that it was probably cancer. I found it incredibly sad for that person and his family. As best I could imagine how they felt and therefore I could comfort them, distract them when necessary and give them guidance in the medical world. It sounds weird but I felt useful. Something I do not often feel. It did me good. It is not ruled out that it is cancer, although the probability of it is quite small.
I'm probably just tired, which makes me more susceptible to stress. This week I cycled 125km, with a round just north of Amsterdam with wind gusts as hard as level 5 and I cycled all the hills of the Utrecht Rim, I did not know there were so many .... tomorrow I will cycle in the dunes, 60km with Austria specialist. Create some endorphins, which should give me a more positive feeling. Every day is one day closer to June 15. Bring it on.

Daarnaast komt 15 juni met rasse schreden dichterbij. 1 jaar geleden. Dat valt niet mee. Ik ben in gedachten al bezig met een blog voor rond die tijd. Een knal blog. 6 juni heb ik weer CT scan. 10 juni uitslag. Ik voel de spanning nu al. Veel te vroeg. Hoe lekker was de tijd in de Amerika. Geen stress, geen angst.
Een andere factor die bij mij stress oplevert wordt veroorzaakt doordat ik me voor het eerst op internet heb verdiept in mijn ziekte: slokdarmkanker. Waarom? Omdat jullie mij de Mont Ventoux op gaan sponsoren. De opbrengst gaat naar slokdarmkanker-onderzoek. Maar als jullie mij sponsoren, moet ik jullie wel kunnen vertellen wat slokdarmkanker is, bij wie het voorkomt, wat de prognoses zijn. De cijfers zijn wel om ongelofelijk depressief van te worden......
Ik heb net ook meegemaakt hoe het is om aan de andere kant van het onheil te zijn. Bij iemand in mijn omgeving werd mogelijkerwijs kanker geconstateerd. De persoon moest een CT scan ondergaan terwijl op het verwijsbriefje stond dat het waarschijnlijk kanker was. Ik vond het ongelofelijk vervelend voor die persoon en zijn naasten. Ik kon me als de beste voorstellen hoe zij zich voelden en er daarom voor hen zijn, hen afleiden wanneer dat nodig was en hen wegwijs maken in de medische wereld. Het klinkt raar maar ik voelde me nuttig. Iets wat ik me niet vaak voel. Het deed me goed. Het is nog niet uitgesloten dat het kanker is, hoewel de kans wel vrij klein is.
Ik ben waarschijnlijk ook gewoon moe, wat me vatbaarder maakt voor stress. Deze week 125km gefietst, een rondje Waterland met windkracht 5 en alle heuvels van de Utrechtse Heuvelrug op gefietst, ik wist niet dat dat er zo veel waren....morgen in de duinen fietsen, 60km met de Oostenrijk specialist. Endorfine aanmaken om mij een meer positief gevoel te geven. Elke dag is weer 1 dag dichterbij 15 juni. De beuk erin.

50.000
Ik wil jullie allemaal bedanken voor het trouw lezen van mijn blog. Ondanks mijn stilte afgelopen weken hebben we de magische grens van 50.000 page views onlangs gehaald. Dat klinkt veel en dat is ook veel. Page views worden geteld op basis van views en niet op basis van unieke bezoeken. Ongetwijfeld dat de helft van de page views veroorzaakt is door mensen die kijken of ik weer een nieuwe blog heb geschreven. Een paar interessante statistieken:
- De drukste maand was Augustus met meer dan 6.000 page views
- Gemiddeld zijn er net iets minder dan 150 views per dag
- Ongeveer de helft van de page views komt uit Nederland, een kwart uit Singapore. Na een sterke start is Japan helaas gezakt van een derde plaats naar een meer gelijkwaardige positie gezamenlijk met de VS, het Verengd Koninkrijk en Frankrijk
- Twee-derde van de bezoekers gebruikt een Apple instrument (Mac, I-phone of I-Pad)
- De helft van de bezoekers bekijkt de blog met een mobiel instrument
- Het gemiddelde salaris van een bezoeker is .......:)
Het aantal page views heeft mij doen denken....had ik dan toch reclame moeten toe laten op mijn website...ik had wat geld kunnen verdienen voor een goed doel zoals kankeronderzoek. Misschien toch niet.
Begin juli ga ik de Mont Ventoux op fietsen met een veelzijdige groep bestaande uit mijn zusje, broer, een vriend uit Nederland, een vriend uit Singapore, hopelijk mijn zwager en wellicht nog anderen. Ik moet de sponsorformule nog bedenken maar hierbij al een waarschuwing aan een ieder: Trek uw portemonnaie en sponsor mij voor het ophalen van geld voor kankeronderzoek terwijl ik de Mont Ventoux op fiets!
We zijn net terug na 2,5 week VS. Het was genieten tot en met. Heerlijke quality time met de familie. Een paar dagen LA, genieten van Disneyland en het Californische strand; Las Vegas, een moetje om de camper op te halen (wat een vreselijke stad - wel nog in Ceasar's Palace geweest - helaas was hij er zelf niet); toen in de camper via Zion Canyon - hoge rode bergen om je heen; via Bryce - een maanlandschap bestaande uit roze stalagmieten - naar Lake Powell - een groot meer met daar achter hoge Monument Valley-achtige bergen; vervolgens de Grand Canyon - te groot om te bevatten, lekker omgekeerd wandelen, eerst naar beneden en dan pas naar boven ; via Sedona - een stad met een sterke aarde Vortex- naar Wickenburg (Arizona), waar we als cowboys een paar dagen paard hebben gereden en kleiduiven hebben geschoten; als grande finale 3 dagen NY - Central Park, shoppen, musical, leuke wijken, buzzing city.
Een veelzijdige reis, hotels, camper, appartement, zon, zee, wandelen, shoppen, woestijn, stad en bergen. Zeker de camper is ideaal met kinderen. Je hebt alles bij je en je doet de deur open en de kids gaan op avontuur. Lekker BBQ en genieten van de natuur. We zijn altijd verrast hoe aardig de mensen zijn in de VS; hoewel NY wel wat anders is. We hadden er graag gewoond.
In de afgelopen 2,5 week heb ik zo goed als nooit aan mijn ziekte gedacht. Het was er gewoon niet. Ik was heel even een normaal mens. Ondanks de jet-lag voel ik me nu sterk. Ik moet ongelofelijk hard gaan trainen komende weken om de Mont Ventoux hard op te kunnen. Een nieuwe focus, een nieuw doel. Volgende week dinsdag chemo. Mijn laatste bloedwaardes waren nog nooit zo laag - 4 - zouden ze weer lager kunnen????? Over een paar weken (15 juni) is het al weer een jaar geleden dat deze ongein begon. Wat gaat de tijd toch snel. Wat is er toch veel gebeurd. Wat heb ik toch genoten. Nog even en ik heb de initiële prognose van 8 tot 11 maanden verslagen; nu nog de rest van de probabilistisch Gauss-kromme aan gort maken en een miraculeus record vestigen. Ik heb er zin in en ben er klaar voor. De beuk erin.

I want to thank you all for reading my blog so often. Despite my silence in recent weeks, we have reached the magic number of 50,000 page views last week. That sounds like a lot and that is a lot. Page views are counted on the basis of views and not on the basis of unique visits. No doubt that at least half of the page views are caused by people who are curious to see if I've written a new blog. A few interesting statistics:
- The busiest month was August with more than 6,000 page views
- On average there are almost 150 views a day
- About half of the page views come from the Netherlands, a quarter from Singapore. After a strong start Japan dropped from third place to a more equal 4th position jointly with the U.S., the UK and France
- Two-thirds of the visitors are using an Apple device (Mac, I-phone or I-Pad)
- Half of the visitors view the blog with a mobile device
- The average salary of a visitor is ....... :)
The number of page views made me think .... should I have allow advertisement on my site? I could have earned some money for a good cause such as cancer research. I am not sure whether I should....
Early July I will cycle-up the Mont Ventoux with a diverse group of friends and family, consisting of my sister, brother, a friend from the Netherlands, a friend from Singapore, hopefully my brother in law and possibly others. I am still not sure about the sponsorship format but here already a warning to everyone: Pull your wallet to sponsor me to cycle up the Mont Ventoux to raise money for cancer research!
We have just returned from a 2.5 weeks trip to the U.S.. It was great. Incredible quality time with the family. A few days LA, to enjoy Disneyland and the California beach, Las Vegas, we had to go there to pick up the camper (what a terrible city - I went to Ceasar's Palace - unfortunately he was not there), then with recreational vehicle via Zion Canyon - high red mountains around you, to Bryce canyon - a lunar landscape of pink stalagmites - onwards to Lake Powell - a large lake with behind it high Monument Valley-like mountains, then the Grand Canyon - too big to contain, inverted hiking, first down and then up, through to Sedona - a city with a strong earth Vortex and onwards to Wickenburg Arizona, where we played cowboys for a few days and did some skeet shooting, and as grand finale 3 days NY - Central Park, shopping, nice neighborhoods, buzzing city.
A versatile vacation, hotels, camping, apartments, sun, sea, hiking, shopping, desert, city and mountains. Certainly the camper is great with children. You have everything you need with you and you open the door and the kids go on an adventure. Tasty BBQ, while enjoying the nature. We are always surprised about how nice people are in the states; although NY is somewhat different. We would have loved to life there.
In the past 2.5 weeks, I hardly ever thought about my illness. It just was not there. I was just a normal person for a while. Despite the jet lag I now feel strong. I have to train incredibly hard in the coming weeks for the Mont Ventoux. A new focus, a new goal. Next Tuesday my next chemo. My last blood levels have never been so low - 4 - could they be even lower again??? In a few weeks it's been a year since this crap started. How quickly the time has gone. How eventful has it been. How I have enjoyed myself despite everything. Pretty soon I will have beaten the initial forecasts of 8 to 11 months, provided the chemo would work (without 3 to 6 months). It is time to beat the rest of the statistical Gauss curve and set a miraculous record. I'm excited and ready for it. Bring it on.

Begin juli ga ik de Mont Ventoux op fietsen met een veelzijdige groep bestaande uit mijn zusje, broer, een vriend uit Nederland, een vriend uit Singapore, hopelijk mijn zwager en wellicht nog anderen.
