Alles goed - All Good

Goede bloedresultaten. Tumor markers weer naar all time low - 7 - reeds eerder gehaald in januari na skiën.

Lekker, Ik heb me vergist.

Good blood results. Tumor markers back to all time low - 7 - I previously reached this level in January after the skiing holiday. They have given me an extra week of rest for my hands. The pain was now continuously, on a scale of 1 to 10 a 4; to be frank it starts to be unbearable.  I can barely stretch my fingers, there is pus coming from under most of my fingernails, my skin is very thin, my hands are  extremely sensitive and my fingers start to resemble that of ET, with an extra thick last phalanx.

An extra week of rest will let them heal. This good because I will be pill free when skiing with my sister and friends. Today I still go some bevacizumab. It always feels good to fight that damn cancer. Friday we leave for Switzerland. I'm really looking forward to some skiing.

It remains strange what game your brains plays with you before a blood test/CT scan. Sunday I went to bed early because I thought that I would have chemo on Monday. When I turned out the light and was ready to sleep I realized that something was wrong. The Nurse Practitioner with whom I had an appointment with does not work on Mondays....... Do I really have chemo tomorrow? A quick check...damn no, I had mixed up the dates. Not so nice, I had a companion who would pick me up at  730, I had written it on my blog .... Chemo destroys more than it should ..... my brain capacity is deteriorating very quickly. I need to do something about this.

I was sad yesterday in anticipation of the test of this morning but felt nevertheless pretty good. Oene and Eline were already awake at 6 am this morning, me to, it was a treat to have them in bed with me for almost an hour. Life can be great!

Even van me af schrijven - Releasing some steam by writing

Mijn trouwste belangenbehartiger kon tijd vrij maken. Mijn schoonzus kon me brengen.....alles was geregeld. Ik sliep goed maar kort dinsdagnacht. De dag vloog voorbij. Mijn schoonzus haalde me op. Ja ergens laag in mijn darmen. Bring it on. De beuk erin.

Stay strong.

I will beat the statistics.

Finally someone who saw something positive in my case. My most loyal advocate had asked twice if they had all information necessary. Yes, they said. I also asked. I should not worry, they knew my case. They had all the information they needed. They were specialists in removing the oesophageal. They did it 40 times a year. Yes, you could well live without one. You just had to adjust your eating pattern. Your stomach would take over the function of esophagus. A tube instead of a bag. Small amounts of food. Sports would not be a problem. The operation was typically carried out with a robot. It would be done maybe already in seven weeks, that would be enough time after the last chemo. No, they had not operated on patients with similar severe metastasis, but that did not matter. The standard had shifted. Yes, they had all the info, it would only be necessary to do a PET CT scan. If this was no worse than that of June if would be a go ahead. If i could drop by as soon as possible to speak to the doctor. The PET and CT scan and results session could still be arranged before my skiing holiday. Sure I could drop by tomorrow. I'll be there at half past four. This was Tuesday 5 o'clock. The conversation lasted nearly half an hour. Suus was still sick ... who would accompany me? My parents would take all the grandchildren to a Dutch children show, so they could not do it. My most loyal advocate could make time. My sister in law could bring me ..... everything was arranged. I slept well but briefly Tuesday night. The day flew by. My sister in law picked me up. We had plenty of time. They let us wait for more than an hour. The interview went well. All questions were answered. It was to good to be true. Yes, other hospitals had said no. It happened. They saw it differently .... they were the forerunners. My reaction to the chemo was unique. Yes, the only thing was that still missing was the first PET CT scan in June had viewed. Or else I could mail them the pictures. To what level was the metastases? Eh....somewhere in my intestines up to the level of my navel. No problem as long as it was not lower. I would be called today (Thursday). There was no indication that I might get bad news. At 13.10 the doctor phoned me. Ehh, he had not realized that my metastases was not only around my esophagus and my intestines. My metastases are also around the aorta, my heart, my pancreas etc yes that was still different. They were sorry. Surgery is not an option. Goddamn ........ bastards. I'm not a plaything. You said you had viewed all the info. Worst of all my most loyal advocate and I devised our own protocol to prevent these kind of mistakes from happening. We had learned it the hard way in December with all the second opinions. They cost a lot of energy. Only when the doctors confirm that they have all the info, then I come into the picture. Beforehand all contact go through my most loyal advocate. He is not to blame. They confirmed to him twice that they had viewed everything. Luckily I only walked around for 48 hours with the idea that we had the solution. Suus is still sick, so the kids and running the household are a good distraction. Monday my next chemo. I'm even more motivated than before Tuesday. Bring it on. I am ready.

Stay strong.

I will beat the statistics.

The other side of the coin

No idea why, but this time I feel more like starting in English. It is a more subtle language than Dutch, which I am forgetting quite rapidly with all the Dutch around me and the chemo eating up my brain.

Rome was great. Suzanne suggested we rent a scooter, which we did. No need for any drivers licence in Italy up to 125 cc. We drove more than 85 kilometres within Rome in 3 days. We saw everything and enjoyed the Roman way of life to the max and the heavy traffic in Rome. At the end of the 3 day trip I drove more Roman than Roman do. I totally ignored my dietary requirements and enjoyed the Italian food in all aspects... anti-pasta, pasta, pizza, primi, secundi, fish, saltimbocca, truffle, ice cream (the best in the world - chocolate meringue!) and desserts. I even drank two small glasses of wine - Barolo off course! I brought back with us a big shopping bag full of Italian specialties, including some tomatoes, red pepper and truffle.

We returned Sunday afternoon and had the result appointment on Monday morning. I slept fine, still in my Roman blues. I was quite relax while awaiting the results. Within a minute while telling the doctor how many kilometres I had biked, skated and skied, she gave us the good news that all was stable. Normally she is very conservative and careful with her remarks, but this time she showed for the first time that she also is starting to believe in a miracle. She briefly mentioned that maybe in a few month time they could consider other avenues......is this light at the end of the tunnel? For the time being she was very clear not to change the winning strategy; i.e. we are proceeding as planned.

It is interesting to experience that although Suzanne and I were very relax beforehand, still in the Roman blues probably, underneath there was a lot of anxiety and stress. It all erupted afterwards.....suddenly we were in a stupid fight and before I knew it I was in the train heading home while Suzanne was in the car. Not something to be proud off. But the fact of life is that this disease causes a lot of stress in a relationship. We both experience it differently and more importantly, our feelings and anxiety will be different at any given point in time. Our feeling will not often be in sync. I can be in low while Suzanne is more stable or vice versa. The thing is that you will be protecting yourself and will be ensuring that you do not slip into the low together. We were there during the first few days and I can tell you it is devastating; as such you go in survival mode and do not want to go down at the same time. It is of course also very different for the patient and the care taker. In any event it is very strenuous on a relationship. Thank god (if there is one) that despite our ups and downs (which are clearly more caused by my volatile personality) we are also able to find time and space to level and share our feelings; to be positively in sync. This ensures that we are still able to enjoy life. It is easy to drift apart as a result of cancer. Unfortunately, it often happens that people split up after having recovered from it. I do not expect this to happen to us, but there is no free lunch in life, we will need to work on our relationship to ensure that it won't happen.

It might sound strange but I am sometimes getting very tired of my way of life. I am constantly trying to convince myself that I will not die. 62 rounds on the skating ring, 8000 metres of altitude difference on the ski slope, 32 km on natural ice and most recently 37 km on the mountain bike. Each time longer, harder and deeper. Not to convince you, but to convince myself; I will not die. I will win this fight. If I am at the top of world while riding my bike, skiing down the slope or skating on ice, I will not die. Even more, nobody around me will ever be able to grasp that I am doing this while having cancer. They don't know it and considering that I am overtaking them on my bike or on ice, or skiing smoother and quicker down the hill they will never expect it. If I sport alone I typically am shouting and screaming at the cancer while doing my thing. Most people that come across me will think "that guy is crazy". Although all the activity gives me a boost in endorphines, it is tiresome to always have to perform. I do not need to win from the people around me but I have to out beat myself. I have to win. My drive to live is fueled by my anxiety to die.

I am greatly jealous of you out there that are living my live that I always wanted to live. That's why I might not always sound that interested in your stories. I live in my own world. A very different one than yours. One full of drive, joy, stress, anxiety and cancer. That's my live. Your's feels like history for me for the time being.

This weekend it was my 39th birthday. Despite that Suzanne was sick (and is still in bed with flu) I enjoyed a great day with my family. We went to watch Ajax in the Amsterdam Arena with almost my entire family. They did not win but it was a great experience. I was a bit anxious to take my entire family to a place full of hooligans, but luckily we were sitting opposite the hard core fans instead of in their middle.

You can hear Oene cheering for the biggest Dutch soccer club!

Tomorrow a last minute second opinion. A Dutch specialist of oesophagus resection who sounded positive. Let's keep our fingers crossed. This might be it. This might be the break I have been waiting for......if not, Monday yet again a blood test and Bevacizumab, the start of my 11's chemo session. Bring it on. I am ready. The week thereafter 10 days of skiing with the family. Than a few days at home and again a short ski trip withmy sister and a few friends. Hard core skiing (just so they know). I might have cancer, but at least I ski 4 weeks this year.

Stay strong.

I will beat the statistics.

Het menselijke brein - The human brain

Slechts 1 keer eerder hadden ze heel even geschaatst.

Mijn gewicht is goed. Positive thinking, self-reliance and leave people and things better than you found them. De beuk erin. Het komt goed.

Stay strong. I will beat the statistics.

How beautiful the Netherlands can be in winter. I have greatly enjoyed recent days. Last Friday I went ice skating on natural ice for the first time in years. I did that in the north of Holland,  in Friesland on the Nannewiid. What a delight. Below some pictures.

Yesterday I went skating with Oene and Eline on a small lake near Bussum. Their first experience on natural ice. They really impressed me. They had only done it once before for 20 minutes. Despite the fact that they were falling every other minute and their lack of proper skiwear they kept going. I was and am very proud of them. Today I'm going to do the same with Fleur.

This afternoon I have another CT scan. Thursday Suzanne and I will leave for Rome, we return on Sunday and Monday we will get the result of the CT scan. It's nice to go to Rome and to have some distraction to ensure that I do not think too often about the result times. I would love another ticket for 9 weeks to live. Okay, I will need to go to the hospital every three weeks for chemo and a blood test, an intermediate test, but the CT scan is the real deal. Three weeks back I was tremendously worried about my blood results, I had aches everywhere and my brain translated it into "probably bad news....". It was wrong! My blood values were never so good. My tumor markers  dropped from 9 to 7 (they were once 132), if they are under 6 you are considered healthy.

In recent weeks, the realization occurred to me that our lives will never be the same. In banking terms I will never have a 'risk free' life. The carefree and reckless life is over. There will always be fear for what may come. I used to  dream about "later when I am old .... " or to plan vacations months in advance, that will not happen. Not anymore.

It's interesting how the brain responds to fear. Despite the uncertainty, I manage to enjoy every day. There are even days that pass in which I do not think about my illness or fear. Our brain is capable of making that fear so small and to put it so far away that you can live with it and enjoy life. It's weird to realize this. Am I honest with myself? Should I not think more often about it? No, if you want to survive and thus want to enjoy what is to come, you have to isolate the fear and uncertainty, to bury it, to exclude it from your life. It's like some feelers inside me are off. You take as much control as possible over your own feeling to ensure that you can live the life that you want.

Once every three weeks I go to a psychologist. During those conversations I sometimes open doors behind which I have buried that fear deep in side my brain. It's nice to be able to let go in a protected and closed off environment. Tears flow heavily. The funny thing is that I talk a lot more about the fun things I've done and enjoyed than about the fear. Every time I realize how lucky I am, that I have been able to enjoy the last few months. I always go away with a good feeling.

My illness has brought a lot of good things. Besides that I spend a lot more time with the kids and live a life closer/more honest with myself, I enjoy much more the here and now. I think more often back about today and realise how beautiful it was. A conscious life. Live in the now instead of in the future. Singapore was for me a bit of the 'lala land' (a remark I learned from a banker from Deutsche Bank when discussing one of my more difficult local Singaporean client), it is also called a dream world or a pink cloud. Always busy with fun things. Always better, longer and bigger things and experiences. Often too little realisation and awareness of the here and now. The funny thing is that I only now, since I have more time,  really begin to miss Singapore. The carefree life. Recently I had dinner with 2 good friends from Singapore i.... it felt familiar as if nothing had happened. As if tomorrow I will step on the boat in Phuket.

I have difficulty sleeping the last few days. I have huge muscle pain in my shoulders. So bad that I wake up at night. My calves are tight with tension. Is it something or is it stress? Is it my brain that fools me? So I am not 100% in control of my brain ..... Suzanne has the fullest confidence in me. According to her, I look good and healthy. My weight is good. I have done a lot of sports and enjoyed the outdoors. I had a lovely dinner with a nice colleague from Singapore. Anyway, Monday we'll know. It should be good. It must be good. Positive thinking, self-reliance and leave people and things better than you found them. Full power. It will be good.

Stay strong. I will beat the statistics.

In transit

Mijn vader was ooit Nederlands afdalingen kampioen.

Hoe moeten we nu ons leven invullen?

Stay strong.

I will beat the statistics.

What a holiday! We had 2 fantastic weeks in the alps, snow, sun, fun and wonderful family around us. Absolutely fabulous. Fortunately, because I had so longed for it in the past months. The idea of skiing was a way for me to get through the tough months. To have something to look forward to. The danger of longing for something too much is that it will be a disappointment, especially if you have envisioned how it will be for such a long time.... but not this time. It has surpassed my wildest expectation.

Referring to my last bog, the funeral of my grandmother was very beautiful and dignified. For me the church was quite emotionally. I think churches are great places to unwind, the ritual of a Catholic mass is familiar, the incense smells great, the singing and the standard texts are familiar. But .... what is said about life only being an intermediate stage before something more beautiful I find hard to accept. Yes, it's a great idea to be able to envisage that there is something after death, but damn this is the real thing. This is when and where you need to do it. Where you should enjoy to the fullest. Where you need to think about what you have done and want to do and leave behind. This is the life.

Enough spiritual thoughts for now, back to the wonderful skiing holiday. First week with the parents of Suus. The week of Christmas is always relatively quiet in a ski resort. The kids had more or less private lessons. Eline and Fleur were each with 2 others in a group. Oene had a more cultural experience with 7 French kids in his class ..... especially since the class was 3 full days long including lunch. It snowed a bit an even rained once but overall it was a lovely week in which Suus and I also enjoyed a couple of full days skiing. On Saturday, Fleur, Eline, Suus and I made the crossing from Morgins to Morzine. It turned out to be a  relatively simple trip without any difficulties for the ladies. We arrived at 13.30 in Morzine, after a long and healthy lunch in the sun.

We were 19 (my whole family - parents, 2 brothers and sister including spouses and kids) in a chalet on the piste. Absolutely marvellous. In my family, the focus is on skiing and it is pretty competitive. My father was once the Dutch downhill champion. Every morning, a group of adults left at 8.30 for an early morning of skiing. Often we were earlier on the piste than most lifts opened. But what a pleasure to be the first on the newly groomed slopes rushing down the mountains. In a morning we would ski  more than most would o in a day or perhaps a week..... As said, there is a fair amount of competition in our family. Each track is a kind of race.. great fun! My fastest downhill time this year was 138.9 km / hours.....at noon we would pick-up the kids, have lunch with the entire group somewhere on the mountain in the sun. The afternoon was spent skiing with the family. The children won many medals this years. Soon they all will have finished their classes and be ready for the real races. Last Friday we completed a black slope with the 5 of us. Eline and Fleur as routinees, Oene with a little more effort. That same day I had to admit that Fleur was faster down an icy slope piste than her old dad.... nice to experience as a parents, that your children are better than yourself in certain exercise. Oene also learned some french..... non, non, non, non, attention ..... with a disapproving finger of the teacher including.

On the first of January at 10:15 I had already skied twice off the famous Portes du Soleil wall. I also lost my phone that day on the slopes. I found him back a half inch of a ravine ... if this is not a good omen for 2013 I don't know it. On Wednesday my brother, brother in law and I hired a guide. We made 5 fantastic off piste descents in 30 cm of fresh powder, climbed a steep cliff of 100 meters up (do not look back and for sure do not fall), skied through dense forests like in the old James Bond movies. We have nearly descended in total 8000m altitude meters in one day. I'm still tired ...... but I survived that day and was not less physical shape than the others......in fact I believe I was the strongest that day. I have rarely though so little about cancer or being ill as that week.

And then we came back to Bussum ..... although we had a lovely Sunday, picked-up our dog Jesse (he had used their living as dog outlet) and went for a short walk through the woods with friends and had a lovely dinner, Monday was really very challenging. No, it was not the children, they feel at home at school and even looked forward to returning .... but for me and Suus. I walk through the school and although I now know a few parents, I feel like an outsider ... what do we have to offer except whining and sadness? This applies not only to the people at school but just as much to the people in our neighborhood. We have no new social life. We do not have the energy and I doubt whether we are going to ever have it. I am happy that occasionally I am being copied-in in emails from Singapore, where I deliver unsolicited comments to the plans of 10 of my friends to do the jungle triathlon.....what a nonsense.

The question also arises to Suus and me, what should we do? How should we fill in our lives? Should I go to work cautiously, should Suus go back to studying or to orientate herself to restart working? Should we invest in a new social life? In short our life feels like that of a traveler in transit waiting for his next flight .... the big difference is that we do not know where are flight is heading and how long the journey will take. Sometimes this feels very annoying and sad ..... last Saturday we should have had the Emirates flight from Geneva to Singapore instead of at the same time the Transavia to Rotterdam. I'd really like to have taken that flight to Singapore. Today I had the result of my last second opinion. Same story as before. I've had it hearing that it's coming back and that I will die. They do not get it. I'm not going to die. I am going to beat the fucking disease.

To the hospital tomorrow for antibody and blood tests. It will be damn good. Fuck it. Try to stay positive and keep believing in it.

Stay strong.

I will beat the statistics.

Happy 2013

Het is even stil geweest op mijn blog. Waarom?

In haar speeches zaten altijd duidelijk boodschappen verwerkt. Ze hechte heel veel waarden aan familie. Wij respecteerden haar enorm.

Stay strong. I will beat the statistics.

It's been quiet on my blog. Why? December is always a busy month in the Netherlands. Sinterklaas (Dutch traditional family holiday), family birthdays, Christmas and a weekend away with Suus and the children. We also obtained a lot of second opinions.

I actually wanted to dedicate my next blog on how cancer changes you, but unfortunately I did not have sufficient time for such blog. I will prepare it in January because you can bet that cancer changes you, whether you like it or not.

We had a wonderful Sinterklaas with the 5 of us. Suus and I went to Maastricht, the most southern city in the Netherlands. My mother's family comes from Maastricht. It is not a very Dutch city, people are friendly and look smart. The food and restaurants are great. A city that I love and in which I feel quite at home. Suus and I slept in an old monastery and enjoyed a fancy dinner in a trendy restaurant. I did not know that cucumber mouse could taste so good! The next day we had our annual Sinterklaaslunch at my grand mother's home. We call her Bonnemaman. We were with 60 people. The highlight is the gift room, where each of Bonnemaman's children has a table filled with small presents and chocolates for each of its children, grandchildren and grand-grandchildren. Bonnemaman enjoys the spectacle to see how surprised everyone is how generous Sinterklaas has been that year.

Bonnemaman has 4 children, 2 who live in the Netherlands and 2 who live in the French speakng part of Belgium. Together it is a diverse group where everyone is different but where everyone knows each other very well and understands each other. My grandmother used to hire (up to 4 years ago) every summer (since the 70s) a large villa on the Belgian coast (Knokke Le Zoute) during the whole month of July where we celebrated our summer holidays. Knokke Le Zoute is formerly a fashionable seaside resort full of old Belgian (French-speaking) families, now at overtop town full of Russians and loud mouthed Dutchies. When I was young I stayed there for a month and later on as a student and while working I would stay there a long weekend. The villa was huge; we could stay ther comfortably with the 26 of us. There were strict rules. My grandmother always sat at the head of a long table, the other sat next to her in order of seniority. I, as one of the younger ones, was always miles away from her or even at another table. I have endless good memories of my holidays there with the entire family. We did a lot of sports, played games, went to the beach and always watched the Tour de France with the cousins.

Last Sunday, the day after the lunch we went visited her and ate a piece of southern Dutch cake. She still lived on her own in the center of Maastricht. My grandmother had the gift that she could let everyone feel special. I have always been convinced that I was her favorite grandchild. No doubt that the other 13 grandchildren share that same feeling. Every 5 years we would celebrate her birthday with a big family reunion. She would also speech at the diner. Half of the speech was given in French (her mother tongue) and the other half in Dutch. Her speeches always included clear messages family values. We respected her greatly.

Bonnemaman died last Friday suddenly, a short week after the lunch suddenly. It happened exactly as she wanted, a sudden death without any illness. She was 92 years old. Last weekend she was hinting that this might be the last Sinterklaas lunch. I then told her that I hoped that I would become as old as she. I really hope so. Her death has triggered a lot of emotions. I'm not looking forward to her funeral this weekend. It makes me think a lot about mine. A morbid thought. The 6 male grand children will carry her coffin in to the Saint Servatius church (me too). Who would carry my coffin when the time comes? Do I want to be buried or cremated? We would actually fly to Geneva Saturday. Our children will leave for Switzerland with Suzanne's parents tomorrow. They will not go to the funeral. Who would actually come to my funeral? How important do I find this? He that is a nice number for at a funeral.....Thoughts that are part of live...or not. I do not know it any more. In any event these are thoughts that make me very sad. Thoughts you never want to have.

This week we had a number of 2nd opinions. The preparation requires a lot of mental strength and results in the (un)necessary stress. The good thing is that the answers are all the same. The pity is that nobody really has a solution at this time. Should I not have gone to these doctors? No, it's good. I want to make sure that I do everything that I can possibly can to bet this  decease. In addition, several doctors now confirmed that my type of cancer (adenocarcinoom) is not caused by alcohol or smoking but is caused by pure bad luck and reflux from the stomach (which I am not sure I had). The 7 reasons mentioned last time are pure bullocks. My response to chemotherapy is "very nice" according to my doctor, which was again confirmed by others. I am even like "a white crow" denoted 1 of the doctors. It is now just time to keep the fingers crossed that the cancer, which the younger you are, the more aggressive it is, stays as long as possible and preferably forever away. Perhaps in 12 months or so  resection is an option but only if it stays away and in any case not now. This message strengthens me back to a strict regime in terms of diet and strengthen my believe to enjoy the next two weeks skiing. First a week in Morgins with the parents of Suus (and 2 days with her brother and his family) and then a week with my parents in Morzine with the whole family (18 pax). I am looking forward to it, even more to the ski trip from Morgins to Morzine which I will undertake with Eline and Fleur.

I wish you all a very merry Christmas, and a healthy 2013. That the damn cancer stays away that whole fucking year!

Stay strong. I will beat the statistics.
